Exercise-induced bronchoconstriction (EIB) occurs when physical activity causes the airways to temporarily narrow. Symptoms, which typically begin during or shortly after exercise, may include coughing, wheezing, and shortness of breath.

Cold, dry air makes EIB more likely, which is why outdoor winter exercise and air-conditioned gyms can be particularly problematic. When a person breathes heavily through the mouth during intense activity, the nose’s natural warming and humidifying function is bypassed, and harsher air reaches the airways directly. Running and cycling may provoke symptoms more than low-intensity activities like walking or swimming.

Physical activity is not off-limits for people with asthma; athletes can manage the condition successfully. The goal is not to avoid exercise entirely. Patients benefit from approaching physical activity with the right tools and a clear plan, so they can remain active without compromising their respiratory health.

How Can a Doctor Help?

A primary care doctor plays a central role in helping patients with asthma stay active safely. The first step typically involves a full assessment of the patient’s asthma severity, triggers, and current medication use, which allows the doctor to build an accurate clinical profile. A personalized management plan is then developed, and for patients with EIB, doctors may recommend using a short-acting inhaler before exercise to prevent symptoms. A doctor can also advise on which types of exercise are least likely to trigger symptoms.

Regular follow-up appointments allow doctors to track symptom patterns over time. If a patient’s condition changes or worsens, the treatment plan can be adjusted accordingly. This helps treatment remain aligned with the patient’s current needs, and consistent monitoring is key.
